WHAT IS REABLEMENT CARE?

Reablement care is a goal-focused form of support that helps individuals regain or improve their ability to manage everyday activities following illness, injury, surgery, hospital discharge or a decline in confidence.

The aim is not simply to complete tasks on someone’s behalf. Instead, carers support the individual to practise activities safely, rebuild familiar routines and become less reliant on assistance wherever possible.

Depending on the care assessment and agreed goals, support may include washing and dressing, preparing meals, moving safely around the home, managing morning routines, completing light household activities and returning to everyday community life.

Reablement care does not replace physiotherapy, occupational therapy, nursing care or other regulated clinical treatment. Where professionals have provided an agreed plan, our carers may support the individual to follow appropriate everyday routines within the boundaries of the care plan.
